Written Verification that Original or Amended Plan has been signed by all parties prior to execution of services

This policy will be reviewed and updated as needed.

PURPOSE

To ensure that plans are signed by both client and counselor before services are rendered by contract providers.

POLICY

DBS Counselor will prepare and submit an email to the local community rehabilitation providers informing them that the IPE or IPE Amendment has been signed by client and counselor and that services may begin.  This email must be copied and pasted into an AWARE case note by DBS counselor. The Contractor is not authorized to provide contract services until this notification is received from DBS.  (Note that this communication excludes Pre-IPE Assessment Services as specified in Attachment F.)
