Apply for Services
There are two forms you must complete to apply for DBS services: 1) the Referral and Application for Services form and 2) the Authorization to Release Information form. Both documents are available for download below in English and Spanish. To download the document, right click the appropriate link and choose "save link as" or "save target as" on the context menu that appears.
