May 2012 Business Opportunity Vacancy Announcements
Please use the Business Opportunity Application (word doc) with the revision date of 1/11 for this selection cycle. In the application, please designate the following:
1. Your complete contact information including current mailing address, daytime telephone number and an email address that you check daily;
2. The Facility number for each of the opportunities your application covers;
3. The Division District Office where you intend to take the selection test; and
4. The testing format you will need.
There are ten (10) Business Opportunities offered in the May, 2012 selection cycle. They are presented by Business Enterprise Region. Instructions are listed below each Business Opportunity.

Absolute Deadline Information
All applications for any or all of the posted business opportunities must be received in the Business Enterprise’ headquarters in Tallahassee by close of business at 5:00 PM on Friday, June 1, 2012 without exception. A post mark is not considered as the receipt date. You will be notified by email once your application is received.

Mandatory Background Screening
All applicants will be required to undergo a Level II Criminal Background Screening pursuant to Chapter 435.04, Florida Statutes immediately upon receipt of the application by the Division. The Division will pay the cost of the background screening. Local transportation for finger printing shall be the applicant’s responsibility.
Should an applicant have a disqualifying criminal history, the applicant will not receive further consideration in the selection process. Such disqualified candidates may request in writing an exception but will not be allowed to proceed in the selection process until a decision has been reached on his or her request.
Vendor Residence Vicinity Requirement
A Licensee who accepts a Type I Licensed Operator Facility Agreement (LOFA) appointment to a Business Enterprise facility is required to establish legitimate physical residency at a distance of not more than 75 verifiable driving miles from the facility.
The Division shall allow the Vendor up to 30 days from execution of the Type I LOFA to comply with this requirement. The Division may grant an extension of not more than 30 days if the Vendor demonstrates a compelling reason for the additional time. Failure to comply shall be considered a material breach of the Type I LOFA and the contract shall be cancelled.
Commitment Information
If the prospective vendor accepts this location as his or her first Type I Licensed Operator Facility Agreement, a one year commitment is required as a condition of acceptance.
Testing Information
The test for the May, 2012 Selections will take place on Wednesday, June 13th, 2012 in the District Office designated by the applicant in his or her application. Testing will begin at 2:00 PM. Persons arriving after that time will be turned away and disqualified from further consideration. The test shall conclude no later than 4:00 PM.
Applicants will be notified by email and follow-up phone call regarding testing date, time, and location. Contact the designated District Office for directions if needed.
Interview Information
The Selection Panel will meet June 26-28, 2012 in the Division’s Tampa District Office located at 415 South Armenia Avenue. You can use a web site (i.e. Map Quest) for driving directions or call the Tampa District Office at (813) 871-7190 for additional assistance.
Interviews for qualified applicants may begin as early as late afternoon on that first day or at appointed times the following days. It is expected that the Panel’s business will conclude no later than 5:00 PM on June 28th if not sooner, depending on the number of applicants qualifying for an interview.
