If you reside in Florida, you can apply for any of our services. Original signatures are still required, so you cannot apply for services on-line. However, you may download the forms and complete them or call your local district office, library, or state office for a form.

- Application for Library Services - Once you complete the application, send it to the Bureau of Braille and Talking Book Library Services.
- Referral and Application Form - Once you complete the application, send it to the nearest Blind Services local office.
- Application for Identification Card - Once you complete the application, send it to the nearest Blind Services local office.
- Application for a Grant Request Form - You must use the guidelines for the Application for Florida Blind Services Gifts and Donations Funds when completing this form.
- Application for a Certificate of Approval for the Sale of Blind-made Products or Services - Once you complete the application, send it to the the Blind Services state office.
- Application for Permit to Solicit Funds or Anything of Value for the Benefit of Blind Persons - Once you complete the application, send it to the Blind Services state office.
If you have difficulty completing forms, call the office nearest you. A Blind Services staff member will assist you in completing any of the applications.
